Control Yuan orders Air Force to make changes after fatal F-16V crash

The Control Yuan has proposed corrective measures against the Air Force -- citing serious shortcomings in risk management and maintenance procedures related to a January training accident that resulted in the death of an F-16V pilot
The jet fighter crashed during a routine nighttime training mission off the coast of Hualien in January.
The pilot's body has never been recovered.
According to the government watchdog, although analysis of the aircraft's flight data recorder is not yet complete, available evidence shows repeated navigation-system malfunctions in the week leading up to the accident.
The Control Yuan says although the aircraft passed safety checks, investigators concluded that the Air Force failed to adequately assess and control the risks associated with conducting a nighttime flight under those circumstances.
The investigation also found multiple violations of maintenance regulations, including failing to complete maintenance records and improperly preserving evidence related to the crash ...

The pilot, Hsin Po-yi was posthumously honored with a presidential citation presented by President Lai Ching-te, who attended his funeral this past Sunday.
